Updates: what changed, when and why
Every recorded change, with the previous value, the new value and the reason. A correction of our own error is marked differently from a later change at the source.
A correction of this site's own error is labelled as a correction. A change made by a provider or by a plan is labelled as a change. The two are never merged, because they mean different things to a reader deciding whether to trust an old figure.

Evidence library: last verified by page
Every page in the evidence library carries its own last-verified date and its review state. A page is published only once a licensed pharmacist has read it against its recorded source (full paper, abstract or publication record, as the page states) and signed it. Pages marked held stay unpublished even after review until their source document has been read directly and the page rewritten.

Last verification by jurisdiction
The coverage record shows a document date and an access date for every province and territory on the coverage page and on each jurisdiction page. Service records show the access date and the recheck assumption on the compare page and on each provider record.
How to read a version
Dataset versions count research merges. Site build versions count generated pages. A new dataset version regenerates all affected pages; a build with no dataset change does not alter any fact.
