The supplied old New Brunswick SA criteria URL returned 404 Not Found; a later web attempt returned Internal Error. A current replacement was found through the official NB Drug Plans formulary page.
New Brunswick current SA criteria PDF is dated September10 2026. Ozempic semaglutide 2 mg/1.5 mL, 2 mg/3 mL and 4 mg/3 mL autoinjectors are listed for adult T2DM added to metformin, or metformin plus sulfonylurea, when glycemic control is inadequate.
NB Ozempic criteria require details of metformin contraindication/intolerance if applicable and a recent HbA1c; inadequate control is HbA1c at least7%. Claims maximum one autoinjector every4weeks.
